Transfer your medications to the mail order pharmacy

Getting your medications in the mail is a two-step process. After enrolling in mail order, you'll need to switch your prescriptions to mail order delivery – instead of picking them up in-person.

Was your medication prescribed by an Advocate Aurora Health Provider?

Option 1: Request new prescriptions in LiveWell
1. Sign in to the LiveWell app or website.
2. In MyChart, select Medications.
3. Select Request refills and choose the prescriptions you'd like to get in the mail, Select Next.
4. Choose Deliver By Mail as the Delivery Method for each prescription. The Advocate Aurora Health Mail Order Pharmacy will be automatically selected.
5. Select or enter your preferred shipping address, then select Next.
6. Review the information show and select Submit.

Prescriptions will show via the medication profile in LiveWell 14 days after the request has been submitted. You'll receive a notification via email or text message when your prescription is being filled and before it ships.

Option 2: Ask your doctor to send your prescriptions electronically

1. Your provider can look up the Advocate Aurora Health Mail Order Pharmacy using the NPI number 1497063895.
2. They can also send the prescription by fax to 262-253-3001.

Prescriptions will show via the medication profile on the LiveWell app 14 days after the request has been submitted. You’ll receive a phone call from the Advocate Aurora Health Mail Order Pharmacy if you haven’t filled a prescription previously with us or if we have other questions. You’ll receive a notification via email or text message when your prescription is being filled and before it ships.

Was your medication prescribed by a non-Advocate Aurora Health Provider?

Ask your doctor to send your prescription electronically
1. The provider can look up the Advocate Aurora Health Mail Order Pharmacy using the NPI number 1497063895.
2. They can also send the prescription by fax to 262-253-3001.

It can take up to 14 days for your request to be processed. You’ll receive a phone call from the Advocate Aurora Health Mail Order Pharmacy if you have not filled a prescription previously with us or if we have other questions. You’ll receive a notification via email or text message when your prescription is being filled and before it ships.

Are you an Advocate Health Care or Aurora Health Care teammate enrolled in the prescription drug plan?

Employees and covered dependents on our prescription drug benefits plan are required to use the AAH Mail Order Pharmacy for most maintenance prescriptions.
